At Terrace Dental Group we value your time and your privacy. We've provided copies of our new patient information forms for you to download, print, and complete at home. You may bring them into our office during your next visit. They will not be stored or sent online, but are offered here simply for your convenience. Terrace Dental Group follows Federal HIPAA requirements for maintaining patient privacy and healthcare portability.

Terrace Dental Group offers payment plans and accepts Visa and Mastercard. We participate with most traditional insurance plans and process insurance claims for ALL patients including plans that reimburse the patient directly. We will do everything possible to help patients utilize their dental insurance to maximum benefits, keeping in mind, that insurance is a contract between the patient, the employer, and the insurance company.
